If you would like to request a duplicate or replacement training certificate you must complete both 
Part 1 and Part 2 of the ordering process.

Part 1 will allow you to use PayPal to submit an administrative fee of $15.00.
Once submitted, this fee is not refundable.

And Part 2 will collect the necessary information to verify your training.

After you have completed both Part 1 and Part 2 our office staff will research your training records and send you the requested certificate.
